Horizons of Hope

A Toolkit for Catholic Parishes on Palliative Care

Horizons of Hope (HOH) is a program to inform parishioners about palliative care, dying, accompaniment and end-of-life concerns and to increase our awareness of our Catholic faith in these areas. It is intended to help each of us – especially caregivers, and family members who need guidance to deal with decisions re wills, power of attorney, and to assist in providing them with community support.

The program was developed under the direction of the Canadian Conference of Catholic Bishops, with strong support and assistance from our Archdiocese. It is comprised of 4 modules:

  • Module 1 – May 23 – 10:00 – 12:00pm the human experience of dying and death
  • Module 2 – May 30 – 10:00 am – 12:00pm discerning/making decisions at the end of life
  • Module 3 – June 6 – 10:00 am – 12:00pm accompanying those at the end of life
  • Module 4 – June 13 – 10:00 am – 12:00pm supporting/integrating within the wider community
